Launching in 2026, Southwest Colorado Cares (SWCC) is a regional Community Care Hub that makes it easier for residents to access the health, housing, and social services they need. By bringing together a network of community partners, SWCC creates one coordinated system where people can find resources, receive warm handoffs, and get ongoing support from Community Health Workers who help them navigate complex systems. Its multilingual online resource directory and closed-loop referral pathways connect families to child care, elder care, food assistance, housing support, behavioral health services, and more. Through this community-wide coordination, SWCC strengthens the region’s safety net and ensures that residents across Southwest Colorado can stay healthy, stable, and thriving.
