What is Southwest Colorado Cares?
Launching in 2026, Southwest Colorado Cares (SWCC) is a regional Community Care Hub that makes it easier for residents to access the health, housing, and social services they need. SWCC brings together a network of community partners to create one coordinated system where people can find resources, receive warm handoffs, and get ongoing support. Its multilingual online resource directory and closed-loop referral pathways connect individuals and caregivers to food assistance, housing support, legal or financial assistance, behavioral health services, and more. Through this community-wide coordination, SWCC strengthens the region’s safety net and ensures that residents across Southwest Colorado can stay healthy, stable, and thriving.
