The Community Connections Board of Directors meets from 2:30 - 4:30 pm on the 4th Wednesday of the month. Meetings are held virtually via Zoom, and are open to the public. Tune in by clicking the button below or by clicking the Zoom link on the meeting agenda.

Community Advisory Committee

The regional Community Advisory Committee (CAC) is being created to advise Community Connections staff on a variety of topics including complaints, policies and procedures, and best practices for case management. To reflect the diversity of our region and clientele, it will be comprised of people receiving long term care services, family members, and professionals from the five counties of Southwest Colorado. If you are interested in serving on the committee please complete the form below. More information will be posted here once the group is formed and officially starts to meet.

Anonymously Report ​Unethical, Harmful or Uncomfortable Behavior

We want to make sure people always feel they can safely report concerns. Community Connections has set up an anonymous reporting line to make it more comfortable to report concerning, unethical, harmful, or uncomfortable behavior involving our staff. We encourage people enrolled into services, family members, contractors and staff to utilize the hotline to report things like:

  • Bias and Discrimination

  • Lack of respect for rights

  • Harassing, threatening, or coercive behavior

  • Misuse of funds or resources

  • Ethical Violation or misconduct

In addition to making your own reports, if you know somebody with concerns that needs assistance to make a report, please support them to do so!

Rights of Persons Receiving Services

You Have Rights!

You have the same legal rights and responsibilities guaranteed to all other individuals under the federal and state constitutions and federal and state laws.  As a person receiving services, you are granted specific additional rights under Colorado State Law. These rights may only be modified by a court order or persons operating under the delegated authority of the court.  In limited circumstances, officials of Community Connections, Inc. hold delegated legal authority to modify the rights of persons receiving services. Community Connections takes our responsibility seriously and commits to use this authority sparingly, transparently, only under circumstances of last resort, and only in a manner consistent with protecting the person from endangering themselves or others. ​​

Your rights are formally described in Colorado Revised Statutes 25.5-10-218 to 25.5-10-235. Community Connections is required by law to maintain a Human Rights Committee to oversee our respect for the above rights. By law, Community Connections can only suspend rights under delegated authority with approval from the Human Rights Committee. No employee or board member of Community Connections shall serve as a member of the human rights committee. Persons receiving services will be encouraged and supported to bring any concerns or grievances related to the respect of their rights to the Human Rights Committee.  Please contact your assigned Case Manager for details about the next scheduled Human Rights Committee meeting.

Notifying the Public of Rights Under Title VI - Community Connections, Inc.

Community Connections, Inc. operates its programs and services without regard to race, color, and national origin in accordance with Title VI of the Civil Rights Act of 1964. Any person who believes that he or she has been subjected to discrimination under Title VI may file a complaint with Community Connections. To request additional information on our Title VI and other anti-discrimination obligations, or if information is needed in languages other than English or alternative accessible formats, please contact our Title VI Coordinator.

ADA Compliance

Community Connections shall make reasonable modifications to policies, practices, and procedures in an effort to ensure Community Connections programs are accessible to individuals with disabilities and to ensure nondiscrimination against persons with disabilities.

Complaints must be filed within 180 days of the alleged discriminatory act. Please call the Office of Civil Rights or submit your complaint in writing to Community Connections’ Title VI Coordinator.
