FSSP PROGRAM ANNUAL EVALUATION SURVEY

This annual survey is required of all community Family Support Services Programs.  It helps both the State and Community Connections know what is going well in the program or not so well, and to learn of any suggestions from program participants that can help us improve.  You are welcome to complete this anonymously (and sending through this website ensures that).  If you care to include your name to allow us to communicate with you about your responses, you may do so at the end.
